If you have misplaced your diploma from PHAA you may order a replacement. Please be aware that the diploma isn't as good a proof of your graduation as your sealed final transcript. So if you only want to prove that you graduated, request a mailed copy of your transcript instead -- it only costs $15.

Write in your First, Middle and Last names below the way they would have appeared on your diploma. Your name and address may have changed since you graduated, so fill in your Current name and the Street and Town-state-zip lines of your current address (where the replacement diploma should be sent).
